S-11045/36/2012/CGHS(HEC) dated 23.10.2023 &FTS NO/FTS NO. 8127278/S-11045/36/2012/CGHS (HEC)/859-70 dated 25.04.2024 the under mentioned QCI recommended / NABH Accredited HCO is being empanelled for various procedure / treatment / investigations under continuous empanelment scheme 2022 with Current status as per terms and conditions of empanelment in the MoA 2022. The package rates to be paid to HCO’s for various procedures / treatment / investigations that have been approved by the competent authority of CGHS will be 90% of the rates approved for CGHS New Delhi & NCR.

Note: It shall be the duty and responsibility of the empanelled Hospital, Exclusive Eye Centre, Exclusive Dental clinic, Diagnostic Laboratory/ Imaging Laboratory/ Imaging Centre at all times, to obtain, maintain and sustain the valid registration. recognition and high quality and standard of its services and healthcare and to have all statutory / mandatory licenses, Permits or approvals of the concerned authorities under or as per the existing laws”. It is requested to the HCO to renew all mandatory documents/certificates timely to avoid de-empanelment.
