Sub: Suspension of empanelment of S.N Dental Hospital & Implant Centre, Shop 5, B-13, Sector-34, Mother Dairy Market, Noida.
With reference to the above mentioned matter, the undersigned is directed to draw attention to the Office Memorandum No. S-11045/36/2012-CGHS (HEC) dated 01.10.2014 vide which S.N Dental Hospital & Implant Centre, Shop 5, B-13, Sector-34, Mother Dairy Market, Noida was empanelled under CGHS and to state that S.N Dental Hospital & Implant Centre, Shop 5, B-13, Sector-34, Mother Dairy Market, Noida was asked to submit reply to a Show Cause Notice issued at above address regarding misbehavior, incomplete treatment and pressurizing a CGHS beneficiary for illegal referral/permission. However, the SCN returned back without delivery and it is learnt that a shop by name Krishna Collections is being run at this address. So it has been decided to suspend empanelment of S.N Dental Hospital & Implant Centre, Shop 5, B-13, Sector-34, Mother Dairy Market, Noida from CGHS with immediate effect for a period of 03 months or till further orders.
The payments of pending credit bills of S.N Dental Hospital & Implant Centre, Shop 5, 3-13, Sector-34, Mother Dairy Market, Noida has also been kept on hold till further order.
