Fortis Escorts Hospital which is empanelled under CGHS Dehradun has violated provisions of MOA-17 despite repeated email and as well as telephonic information regarding expiry of PBG, so far you have not submitted renewed PBG to CGHS. On failure of providing renewed PBG despite several reminders, Fortis Escorts Hospital is hererby De-Empanelled from CGHS Dehradun with effective from 10.12.2021 onwards.
